Adaptive Church Cohort Application

Your application represents a church that is committed to participating in the Adaptive Church Cohort as a team. If accepted into the cohort, your church will be eligible to bring up to 6 people to each of the three in-person summits. This team will need to include at least one senior leader in your congregation (pastor, lead pastor), at least 1 or 2 key lay leaders, and at least 1 or 2 innovator or entrepreneurial types (perhaps a younger leader or someone who thinks out of the box). While you are not required to have this team lined up to submit your application, you will need to have 2 of these 6 people identified to register after your application has been approved, and you will need to have all 6 of these people identified for the first summit.
